Maintaining current contact information is one way to better serve you
 in this time of limited in-person contact. 

Periodically, our parish has important news to communicate such as funeral notices, Mass schedule changes, etc. that necessitate a prompt method of notifying parishioners. It's been a number of years since we collected updated contact information, and our records may not contain the most recent technology method you prefer for contact. 

Parishioner Contact Information Forms are available in the church vestibules, or on this webpage for printing at home.  Give your completed hard copy form to Father, or send via e-mail to [email protected].  Or, simply complete the online version below and click Submit.  

Whenever your contact information changes, please remember to let us know.  Thank you!

Note:  Newcomers are asked to complete the more detailed New Parishioner Registration form, also located in the Registration/Forms section of our website.  Adult children should register independently once they complete their education and join the workforce, even if they continue to reside with you.
